Question asked after Hillary Clinton again cites Benghazi as ‘biggest regret’

Former secretary of state and possible 2016 presidential contender Hillary Clinton has previously cited Benghazi as the “biggest regret” of her tenure in the Obama administration.
Today, at a women’s leadership conference in Boston, Clinton again chipped away at that 2016 obstacle:

But if Benghazi is indeed one of Hillary Clinton’s biggest regrets, it begs for an obvious follow-up question:
If Benghazi is Hillary's "biggest regret", as she claims, then what exactly did she do wrong? Has she been pressed on this?
